August Healthcare in Durham, NC is seeking an Occupational Therapist (OT) to join our rehabilitation team on a full-time basis. You will provide high-quality, individualized OT services to help residents regain function, increase independence, and improve safety.

Qualifications

• Graduate of an accredited OT program.
• Current state OT licensure.

Responsabilités

• Evaluate residents and develop individualized treatment plans
• Provide therapy services in accordance with physician orders
• Document evaluations, treatments, progress notes, and discharge summaries
• Collaborate with nursing and interdisciplinary teams
• Participate in care planning and utilization review meetings
• Educate residents, families, and staff on therapy goals and plans
• Maintain productivity and documentation standards
• Support regulatory compliance and survey readiness
