Discovery Rehabilitation & Living in Salmon, ID is seeking a full-time Occupational Therapist to support our in-house rehab program. You will screen and evaluate patients with cognitive and/or ADL disorders, develop plans of care, and provide skilled, medically necessary therapy.

This role emphasizes evidence-based practice, ADL retraining, and collaboration with physicians, nurses, and therapists. Active OT licensure is required; all experience levels are welcome.

Qualifications

• Must hold an active Occupational Therapist license.
• Open to applicants of all experience levels.

Responsibilities

• Utilize standardized assessments and evidence‑based practice to support clinical interventions.

• Administer ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of assistive devices; etc.
• Assess patient progress and adjust treatments in the clinical record per regulatory requirements.
• Communicate progress and plans with supervisors and other health team members.

Discovery Rehabilitation in Salmon, ID, is a post-acute care facility that provides long‑term care, short‑term care, and geriatric services. We offer a stable and engaging in‑house therapy team and robust programs for our geriatric clientele. We are currently seeking a full‑time Occupational Therapist for our in‑house rehab program.

Duties

• Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan including ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness; modify services in relation to patient evaluation and refer when necessary.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
• Communicate effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
• Provide a comprehensive discharge summary of services per regulatory and clinical requirements.
