Olympia Transitional Care in Olympia, WA is a post-acute facility offering geriatrics and rehab services. We are seeking a full-time Occupational Therapist to join our in-house rehab program and support geriatric patients on their recovery journey.

Responsibilities include cognitive/ADL screening, implementing evidence-based interventions, and coordinating care with a multidisciplinary team. New graduates are welcome; OT license is required.

Olympia Transitional Care in Olympia, WA is a post-acute care facility with long-term, short-term, and geriatric services. We offer a stable and fun in-house therapy team to work with and excellent programs serving geriatric clientele on their journey to recovery. We are seeking a full time Occupational Therapist for our in-house rehab program.

Duties

• Effectively screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders, per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including but not limited to: ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Assess patient’s progress and adjust treatments accordingly in the clinical record, per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provide consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services provided, per regulatory and clinical requirements.

Qualifications

Occupational Therapist license is required.

Open to Occupational Therapists at all experience levels. New graduates are welcome.
