Qualifications
- Licensed Occupational Therapist

Key Responsibilities
- Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
- Utilize standardized assessments and evidence‑based practice to support clinical interventions.
- Provide rehabilitative, skilled, and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
- Administer various procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
- Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness, modify services in relation to patient evaluation, and refer when outcomes dictate.
- Provide consultation and counseling to patients, as well as families, caregivers and other service providers related to functional disorders.
- Effectively communicate with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
- Provide a comprehensive discharge summary of services rendered per regulatory and clinical requirements.
