Innovate Rehab and Wellness in Salisbury, Missouri, is seeking a Licensed Occupational Therapist to join our skilled nursing facility team. This full-time role offers a Monday–Friday schedule with a flexible lineup, focusing on one-on-one therapy to improve patients' functional abilities and independence.

Qualifications

• Master’s or Doctorate in Occupational Therapy from an accredited program.
• Candidate should hold a valid state OT license.
• Experience as an OT is preferred, new grads welcome.

Position Summary

• Evaluate patients’ functional abilities and therapy needs
• Create and deliver individualized treatment plans
• Help patients improve motor skills, coordination, and daily function
• Support patients in regaining independence and quality of life

Key Responsibilities

• Conduct thorough OT evaluations
• Develop and adjust personalized treatment plans
• Use therapeutic techniques and adaptive equipment
• Train patients and caregivers on exercises and assistive devices
• Educate on strategies to improve function and safety
• Collaborate with PT, SLP, physicians, and the full care team
• Document patient progress and treatment changes accurately
• Stay current with OT best practices and research

Qualifications

• Master’s or Doctorate in Occupational Therapy (accredited program)
• Valid state OT license

• Strong communication and problem-solving skills
• Passion for patient-centered care
• CPR certification preferred
