Job summary

Total Care Therapy in Ohio is seeking a licensed Occupational Therapist to evaluate residents and develop individualized plans that improve independence in daily living, upper extremity function, cognition, and safety.

You will provide skilled OT interventions, assess adaptive equipment, supervise COTAs, document in the EMR, and collaborate with an interdisciplinary team to deliver patient-centered care in assisted living and related communities.

Qualifications

• Current State Occupational Therapist (OT) License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
