Job summary

VNA Health Group is seeking an Occupational Therapist to join our team on a per diem basis, providing evaluation and treatment to patients in their homes or community settings in Mercer County, NJ. You will help patients regain independence while collaborating with physicians and caregivers.

The role requires NJ OT licensure, 2–3 years of clinical experience, strong communication, and reliable transportation. A supportive leadership culture and ongoing education opportunities are offered.

Qualifications

• Occupational Therapy Licensure in NJ is required.
• 2-3 years of prior clinical experience and community-based experience preferred.
• Strong computer skills.
• Strong communication and interpersonal skills.
• Valid NJ driver's license.

Responsibilities

• Provide occupational therapy evaluation and treatment to patients in their homes or alternate community settings.
• Assess and evaluate clients for occupational therapy needs and develop action plans with physician collaboration.
• Document changes in client condition and update care plans and outcomes.
• Identify patient, caregiver, and family education needs and tailor teaching strategies.
• Monitor quality and appropriateness of services provided.
• Participate in QAPI and in-service programs.
• Adhere to infection control and safety policies.
• Adhere to organizational policies on attendance and appearance.

Position Summary

Responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings.

Principal Responsibilities

• Support organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.


• Assess and evaluate clients for occupational therapy needs and develops and implements appropriate plan of action in collaboration with the physician.


• Document all changes in client’s condition and updates care plans and outcomes accordingly.


• Identify patient, caregiver, and family counseling and education needs and plans appropriate teaching strategies.


• Monitor quality and appropriateness of services provided.


• Participate in VNAHG’s QAPI and in-service programs.


• Adhere to agency infection control and safety policies.


• Adhere to the organization’s policy in regards to absenteeism and appearance.

Job Competencies & Minimum Qualifications

• Occupational Therapy Licensure in NJ.


• 2-3 years of prior clinical experience and community based experience preferred.


• Strong computer skills.


• Strong communication and interpersonal skills.


• Valid NJ drivers license.

Working Conditions / Physical Demands

Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation.

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $84,000 to USD $112,000.
