Job summary

Ascension St. Vincent's Clay County Hospital in Middleburg, FL is seeking an Occupational Therapist to evaluate patients and design comprehensive plans of care addressing physical, cognitive, and psychosocial deficits, aiming to restore independence and ensure safe discharge.

The role requires a Florida OT license and a bachelor's degree; master's or doctorate is preferred. You will work days with PRN shifts, and benefit from Ascension's retirement plan, EAP, and supportive environment.

Qualifications

• Bachelor's degree in Occupational Therapy is required.
• Florida OT license must be active before/at hire.
• BLS provider within 30 days of hire.

Responsibilities

• Evaluate and design customized plans of care addressing physical, cognitive, and psychosocial deficits to restore independence.
• Implement and adapt evidence-based treatments based on progress and outcomes.
• Educate patients and caregivers on deficit management and adaptive equipment usage.
• Document findings and coordinate discharge planning for long-term success.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Occupational Therapy (OT) credentialed from the Florida Board of Occupational Therapy prior to hire date or job transfer date required.

Education

• Bachelor's degree required. Master's OR Doctorate degree preferred.
