Job summary

Caringhand Home Health Solutions, Inc is seeking an Occupational Therapist for field-based, per diem work delivering in-home therapy. The role involves evaluating home environments, developing and communicating treatment plans, and coaching patients in daily living activities to regain independence.

Responsibilities include teaching adaptive strategies, coordinating with the care team, documenting care, and supporting discharge planning.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the relevant accrediting bodies.
• Registered with the National Registration Examination of the AOTA.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle; ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team for patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition from home health care.
• Perform other duties as required for patient services.

This is a field-based, per diem position that requires travel to patients’ homes. Although listed as “remote,” this is not a work-from-home or telehealth role. Compensation is $130 per visit.

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
