Job summary

Total Care Therapy in Mentor, Ohio is seeking a Part-Time Occupational Therapist to evaluate residents and develop individualized treatment plans that improve independence in activities of daily living, upper-extremity function, and safety.

You will supervise Certified Occupational Therapy Assistants (COTAs), reassess progress, document using the EMR, educate residents and caregivers, and collaborate with an interdisciplinary team to deliver high-quality, patient-centered care in assisted living

Qualifications

• Current State OT License.
• Current CPR/BLS Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized OT plans of care.
• Provide skilled OT interventions to improve ADLs, IADLs, upper extremity function, cognition, balance and safety.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) per state regulations.
• Complete timely and accurate EMR documentation.
• Educate residents, caregivers, and facility staff on adaptive techniques and safety.
• Collaborate with interdisciplinary team for coordinated, patient-centered care.

About the Role As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

Evaluate residents and establish individualized occupational therapy plans of care.

Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.

Assess and recommend adaptive equipment and environmental modifications.

Reassess patient progress and modify treatment plans as appropriate.

Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.

Complete timely and accurate documentation using the electronic medical record (EMR).

Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.

Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Qualifications

Current State Occupational Therapist (OT) License

Current CPR/BLS Certification

Valid driver's license, reliable transportation, and auto insurance

Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
