Job summary

Total Care Therapy is seeking a dedicated Occupational Therapist to evaluate residents, design individualized treatment plans, and improve independence in daily activities. You will work within assisted living and independent living communities across central Ohio.

Qualifications

• Current OT license in the state
• CPR/BLS certification is required
• Valid driver's license with reliable transportation and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required

Responsibilities

• Evaluate residents and develop individualized OT plans of care
• Provide OT interventions to improve ADLs, IADLs, balance, and cognition
• Assess need for adaptive equipment and environmental modifications
• Reassess progress and modify treatment plans as needed
• Supervise COTAs per state regulations
• Complete timely documentation in the EMR
• Educate residents, caregivers, and staff on safety and energy conservation
• Collaborate with interdisciplinary team for coordinated care

Compensation:Per-Visit or Salary (Full-Time Only)

About the Role

As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Qualifications

• Current State Occupational Therapist (OT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
