Advanced Medical Home Care in Minnesota is hiring an Occupational Therapist to provide in-home therapy services, manage a manageable caseload, and participate in a CEU program for ongoing growth.

We offer mentorship, support with documentation, and a Monday–Friday day shift in a collaborative team that values patient-centered care and professional development.

Qualifications

• MN OT license in good standing
• CPR certification for health care providers
• Valid driver's license and reliable transportation

Responsibilities

• Provide occupational therapy services in patients' homes
• Maintain and update charts and documentation for patient progress
• Collaborate with the care team and participate in on-call rotation
