Job summary

Advanced Medical Home Care is seeking an experienced Physical Therapist to deliver patient‑centered home health care in the Forest Lake area. You will bring clinical expertise to patients in their homes and collaborate with a supportive team.

The role offers day shifts, full-time hours, and the opportunity to practice with autonomy while building meaningful relationships with patients and families in their home environments.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Minnesota Physical Therapist license in good standing.
• Current CPR certification for Health Care Providers.
• Valid driver's license to travel to patients' homes.

Responsibilities

• Provide home health physical therapy to patients in their homes.
• Create and manage patient-centered treatment plans and documentation.

Schedule

• Full-time options: .8 FTE (32 hours) or 1.0 FTE (40 hours)
• Monday-Friday, day shift
• 4 bonused on-call Saturdays per year

• Graduate of an accredited Physical Therapy program
• Current Physical Therapist license in Minnesota (in good standing)
• Current CPR certification for Health Care Providers
