Job summary

Home Health Care, Inc. is seeking an Occupational Therapist (OTR/L) to provide comprehensive therapy in patients’ homes across Minnesota. The role emphasizes practical, home-based assessments and tailored treatment plans to restore independence and safety.

Qualifications

• Must have an Active Minnesota OTR/L license.
• Strong clinical reasoning and patient-centered care philosophy.
• Excellent communication and organizational skills.
• Home health experience is helpful but not required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients in their homes and develop individualized occupational therapy treatment plans.
• Improve independence with activities of daily living, home safety, upper extremity function, cognition, and adaptive techniques.
• Recommend home modifications and adaptive equipment that support long-term independence.
• Educate patients and caregivers on strategies that improve safety and function.
• Collaborate closely with physicians and the interdisciplinary care team to achieve outstanding patient outcomes.

Schedule: Full-Time | Monday–Friday

Compensation: $80,000–$150,000 annually
