Job summary

DayToDay Health is seeking an Occupational Therapist to plan, organize, and implement OT services as part of an interdisciplinary team, supporting seniors' independence and safety in the community.

You will assess functional abilities, coordinate adaptive equipment and home modifications, develop individualized plans, and educate participants and caregivers while mentoring OT assistants and contributing to quality improvement.

Qualifications

• Advanced degree in Occupational Therapy (OT) from an accredited program.
• Current state licensure to practice Occupational Therapy.
• Experience with elderly or frail populations preferred; training provided if not.
• Competency in professional practices and adherence to regulatory standards.

Responsibilities

• Conduct assessments of participants' functional abilities, home environment, and rehabilitation needs.
• Recommend and coordinate adaptive equipment, home modifications, and therapy services.
• Develop, implement, and revise individualized treatment plans in collaboration with the team.
• Deliver interventions across various settings including centers, homes, and clinics.
• Educate participants and caregivers on equipment use, safety strategies, and ADL programs.
• Maintain accurate documentation of assessments, treatments, and progress.
• Participate actively in daily interdisciplinary team meetings for comprehensive care planning.
• Supervise and mentor Certified Occupational Therapy Assistants, reviewing care plans and ensuring best practices.
• Train staff on safe OT practices and equipment use.
• Support quality improvement, compliance, and professional development efforts.
