Responsibilities

• Supervise and mentor COTAs and train staff on safe practices.

Position Overview

The Occupational Therapist plans, organizes, and delivers therapy services to support the independence and well-being of participants in a comprehensive, person-centered program.

Key Responsibilities

• Conduct initial and ongoing assessments of participants' functional abilities and home environments.
• Recommend and coordinate adaptive equipment, home modifications, and therapeutic interventions.
• Develop, implement, and update individualized treatment plans in collaboration with the interdisciplinary team (IDT).
• Deliver therapy across various settings including centers, clinics, homes, and contracted facilities.
• Educate participants and caregivers on equipment use, safety strategies, and activities of daily living (ADL).
• Maintain accurate documentation of assessments, treatments, and progress per standards.
• Participate in daily IDT meetings for comprehensive care planning.
• Supervise and mentor Certified Occupational Therapy Assistants (COTAs) and train staff on safe practices.
• Support quality improvement, compliance, and utilization review initiatives.
• Perform integrated functions related to home care coordination and ensure cohesive service delivery.

Qualifications & Skills

• Advanced degree and current license in Occupational Therapy.
• At least 2 years of experience working in interdisciplinary teams; experience with elderly or frail populations preferred.
• Competency in professional practice and ongoing professional development.
• Reliable transportation, valid driver's license, and up-to-date immunizations.
• Ability to pass criminal background checks and meet pre-employment health requirements.
