Job summary

TALENT Software Services is seeking an Occupational Therapist to coordinate restorative and rehabilitative OT services for participants at the Center and at home. You will work with physicians, rehab staff, and the IDT to maximize independence and safety while enhancing ADL performance.

You will supervise Therapy Assistants, train staff and caregivers, manage equipment, and ensure compliant documentation. Strong collaboration with other teams and a commitment to vulnerable seniors are essential.

Qualifications

• Minimum of two years of Occupational Therapy experience in a clinical setting with a frail or elderly population.
• Two years of supervisory experience with demonstrated ability to mentor and develop team members.
• Experience and competency working with people from diverse backgrounds and cultures.

Responsibilities

• Conduct Occupational Therapy assessments to determine care needs, goals, and participate in care planning.
• Deliver and document OT interventions per physician orders, including ADLs and related functions.
• Supervise Therapy Assistants and provide training for Functional Maintenance Programs.
• Provide training to staff, participants, and caregivers in occupational therapy.
• Order, track, and manage equipment maintenance.
• Communicate changes in participant status to the IDT and support care transitions.
• Participate in end-of-life care coordination and support.
• Manage direct reports, including training and mentoring.
• Ensure timely documentation and privacy of information.
• Follow policies, quality improvement activities, and mandatory trainings.

The Occupational Therapist collaborates with the Physical Therapist to efficiently operate the rehabilitation department and facilitates collaborative relationships across the organization and with external stakeholders.

The Occupational Therapist collaborates with the Physical Therapist to efficiently operate the rehabilitation department and facilitates collaborative relationships across the organization and with external stakeholders.

Duties

• Conduct Occupational Therapy assessments to determine care needs, preferences, and goals of the participants and actively participate in IDT meetings to develop participant care plans.
• Deliver and document Occupational Therapy interventions in accordance with physician orders and/or as agreed upon in the participants' care plans, including but not limited to the use of devices, techniques, and activities to promote muscle re-education, perceptual-motor and cognitive function, balance, endurance, and other factors affecting independence in performing ADLs.
• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP).
• Provide training and instructions to staff, participants, and caregivers/families in areas of Occupational Therapy.
• Order, track, and manage maintenance for equipment.
• Remain alert to input from other team members, participants, and caregivers, and update the IDT promptly of any changes in participants' condition or functional status.
• Work with other IDT members to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home).
• Participate in end-of-life care coordination and support.
• Manage direct reports, including training, supervising, and mentoring.
• Be legally authorized to practice in the position location.
• Only act within the scope of authority to practice.
• Complete all required documentation in a timely and accurate manner.
• Protect privacy and maintain confidentiality of all procedures and information about team members, participants, and families.
• Follow organizational policies and procedures and participate in any required Quality Improvement activities, staff training, and meetings.
• Communicate regularly with Supervisor and team regarding workload and priorities.
• Timely completion of all mandated trainings and education.
• Timely completion of all mandated occupational health screenings as needed.
• Exercise flexibility in performing assignments as business needs evolve.
• Other duties as assigned.

Required Skills & Experience

• Minimum of two years of Occupational Therapy experience in a clinical setting with a frail or elderly population.

• Occupational Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.

• Excellent leadership, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members.
• Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.
