Job summary

Live Well Homecare is seeking an Occupational Therapist to plan and implement treatment programs across all ages, working with an interdisciplinary team to promote health and functional independence.

You will assess patients, develop and adjust plans, communicate with families, and contribute to discharge planning while supporting student training and staff mentorship.

Qualifications

• High School Diploma or GED required; Bachelor's degree in Occupational Therapy.
• Licensed Occupational Therapist certification needed.
• Excellent communication, teamwork, problem-solving, and cultural competency skills.

Responsibilities

• Assess patients' performance, develop individualized treatment plans, and adjust strategies based on ongoing evaluations.
• Effectively communicate with team members, patients, and families, respecting cultural diversity.
• Maintain accurate documentation, participate in discharge planning, and recommend adaptive equipment.
• Promote evidence-based practices and contribute to departmental quality initiatives.
• Support student training, mentor staff, and foster positive team relationships.
• Identify and resolve patient and operational issues; engage in professional development activities.
• Assist in departmental safety, infection control, and resource management.
