Job summary

Affinity Home Health Care Inc. is seeking an Occupational Therapist to deliver personalized, in-home and senior living care to older adults using a flexible, patient-centered approach.

You will perform evaluations, develop individualized treatment plans, and provide one-on-one therapy in homes, senior living communities, and virtually when appropriate, collaborating with interdisciplinary teams to improve mobility and independence.

Qualifications

• Valid Occupational Therapy license or eligibility in the practice state.
• Degree from an accredited OT program, proficiency with EMR systems, and current CPR certification.
• Ability to meet physical demands such as lifting, repositioning patients, and performing therapeutic activities.
