Job summary

MEP Health is seeking a licensed Occupational Therapist to provide personalized, in-home and community-based care to older adults, enhancing mobility, independence, and quality of life through evidence-based treatments.

You will perform evaluations, develop care plans, and coordinate with a multidisciplinary team, leveraging telehealth to extend access. A CPR-certified OT with strong computer and EHR skills is preferred.

Qualifications

• Valid OT license in the practice state or eligibility.
• Degree from an accredited occupational therapy program.
• Proficiency with computers and electronic health records.
• Current CPR certification.

Responsibilities

• Deliver one-on-one occupational therapy in private residences and senior living communities.
• Perform evaluations, develop individualized care plans, and implement treatment strategies.
• Support patient wellness, functional independence, and safety through therapy sessions.
• Document care effectively in electronic medical records, ensuring compliance.
• Collaborate with interdisciplinary teams, including PTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff to promote aging in place and safety.
• Incorporate telehealth and virtual care to extend access and flexibility.

Key Responsibilities

Physical & Additional Requirements:
