Job summary

Affinity Home Health Care Inc. seeks an occupational therapist to provide one-on-one, in-home OT for older adults in private homes and senior living communities.

You will evaluate mobility, create personalized care plans, deliver evidence-based treatments, and document progress in EMR systems. You'll collaborate with an interdisciplinary team of PTs, SLPs, and fitness specialists, educate patients, caregivers, and staff to promote safety and aging in place.

Qualifications

• OT license valid and current.
• Degree from an accredited OT program.
• Proficient with EMR systems and CPR certification.
