Job summary

Evergreen Commons Rehabilitation and Nursing in East Greenbush, NY is seeking a skilled Occupational Therapist to join our rehabilitation team. You will collaborate with an in‑house interdisciplinary group to help residents regain independence in ADLs and improve their overall quality of life.

• Current, unrestricted NYS OT license in good standing.

• SNF or long-term care experience preferred but not required - New Grads welcome.

Responsibilities

• Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans aligned with resident goals and physician orders.
• Provide skilled OT treatment including ADL retraining and adaptive equipment recommendations.
• Document all patient care, progress notes, and discharge summaries accurately.
• Supervise OTAs and rehabilitation aides as applicable, providing clinical direction.

We are seeking a skilled and compassionate Occupational Therapist to join our rehabilitation team at Evergreen Commons Rehabilitation and Nursing, a beautiful 242 bed facility in East Greenbush NY! You will collaborate with an in- house interdisciplinary team that respects your clinical expertise to help residents regain independence in daily living and improve their overall quality of life.

Key responsibilities

• Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans aligned with resident goals and physician orders
• Provide skilled OT treatment including but not limited to ADL retraining, environmental modification recommendations, adaptive equipment recommendations, and caregiver education
• Document all patient care, progress notes, and discharge summaries accurately and in compliance with Medicare/Medicaid and facility standards
• Supervise Occupational Therapy Assistants (OTAs) and rehabilitation aides as applicable, providing clinical direction and co-treatment as needed
• Participate in care conferences, MDS assessments, discharge planning, and quality improvement initiatives
• Maintain compliance with all state, federal, and accreditation regulations governing OT practice in long-term care settings
