Qualifications

Position Overview

Occupational Therapy staff members assist with ensuring the health and well‑being of residents by providing comprehensive, quality therapy services. This position is responsible for administering skilled occupational therapy treatments while ensuring the highest degree of quality care is maintained at all times.

About Elderwood at North Creek

Elderwood at North Creek is an 82‑bed skilled nursing and rehabilitation facility located in a scenic, woodland setting near the Hudson River in the North Country. The facility provides a full continuum of care including skilled nursing, long‑term care, subacute rehabilitation, and specialized therapy services. Our interdisciplinary team delivers personalized care in a warm, community‑focused environment. With a bright, spacious rehab gym and state‑of‑the‑art equipment, we offer physical, occupational, and speech therapy for both short‑term and long‑term residents.

Qualifications

• Current New York State Occupational Therapist License (required).
• Graduation from an approved school of occupational therapy (required).
• State license to practice occupational therapy (required).
• Strong clinical, communication, and documentation skills.
• Ability to work collaboratively in a team‑oriented environment.
• Experience in skilled nursing or rehab settings preferred, but not required.
• Two years’ experience in occupational therapy preferred.
• Interest in occupational therapy treatment programs for the geriatric and subacute populations utilizing a team approach.
• Ability to direct others when necessary.
• Effective verbal and written communication skills.
• Current, valid CPR/BLS credential.
• Sense of responsibility and ability to work well with residents, families, and other staff members.
• Ability to work with geriatric and chronically ill residents, as well as other professionals in a friendly and helpful manner.
• Ability to perform tasks to established standards of excellence.
• Proficient reading, writing, speaking, and understanding of the English language at an intermediate or more advanced level.
