Job summary

Capstone Center for Rehabilitation and Nursing in Amsterdam, NY seeks a skilled Occupational Therapist to join our 120-bed rehabilitation team. You will evaluate residents, design individualized plans, and help them regain independence in daily living.

The role involves OT treatments, caregiver education, documentation for Medicare/Medicaid compliance, and collaboration with an in-house interdisciplinary team to support quality of life.

Qualifications

• Degree in occupational therapy from an accredited program.
• Current, unrestricted NYS OT license in good standing.
• NBCOT certification (OTR) required.
• SNF or long-term care experience preferred but not required - New Grads welcome.

Responsibilities

• Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans aligned with resident goals and physician orders.
• Provide skilled OT treatment including ADL retraining, environmental modification recommendations, adaptive equipment recommendations, and caregiver education.
• Document all patient care, progress notes, and discharge summaries accurately and in compliance with Medicare/Medicaid and facility standards.
• Supervise Occupational Therapy Assistants (OTAs) and rehabilitation aides as applicable, providing clinical direction and co-treatment as needed.
• Participate in care conferences, MDS assessments, discharge planning, and quality improvement initiatives.
• Maintain compliance with all state, federal, and accreditation regulations governing OT practice in long-term care settings.

Education

We are seeking a skilled and compassionate Occupational Therapist to join our rehabilitation team at Capstone Center for Rehabilitation and Nursing, a 120 bed facility in Amsterdam NY. You will collaborate with an in-house interdisciplinary team that respects your clinical expertise to help residents regain independence in daily living and improve their overall quality of life
