Job summary

Ahava Therapy Services in Clarksville, TN is seeking a full-time Occupational Therapist to join our in-house rehab team at Ahava Healthcare of Clarksville.

You will evaluate residents, design individualized treatment plans, collaborate with nurses and families, and participate in care conferences to maximize functional outcomes.

This role offers autonomous practice within a supportive setting, continuing education, and opportunities to impact short-term, long-term, and outpatient care.

Qualifications

  • Bachelor’s or Master’s Degree in Occupational Therapy.
  • Active OT license in the state of practice.
  • BLS/CPR certification.
  • Strong clinical reasoning, communication, and documentation skills.
  • Ability to collaborate effectively within an interdisciplinary team.

Responsibilities

  • Review clinical information and evaluate its impact on the assessment and treatment process.
  • Conduct comprehensive functional assessments using standardized procedures.
  • Develop and implement individualized treatment plans with achievable functional goals.
  • Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
  • Revise treatment plans based on patient performance and progress.
  • Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care.
  • Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
  • Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
  • Assist with discharge planning and maintain knowledge of community resources.
  • Participate in care conferences, meetings, and quality improvement initiatives.
  • Maintain clinical competence through continuing education opportunities.
  • Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.

About the Facility

Ahava Therapy Services is seeking a Full-Time OTto join our in-house rehab team at Ahava Healthcare of Clarksville, a 122-bed skilled nursing facility offering short-term, long-term, and outpatient services.
