Job summary

Ahava Therapy Services is seeking a Full-Time Occupational Therapist to join Henderson Health & Rehab in Henderson, TN. You will work within an in-house rehab team to maximize patient independence and function.

Required: OT degree (Bachelor’s or Master’s), active OT license, BLS/CPR. You will collaboratively educate residents and families, document thoroughly, and participate in care conferences and quality initiatives.

Qualifications

• Requires Bachelor’s or Master’s Degree in Occupational Therapy.
• Active OT license in the state of practice.
• BLS/CPR certification (current or during orientation).
• Strong clinical reasoning, communication, and documentation skills.
• Ability to collaborate effectively within an interdisciplinary team.

Responsibilities

• Review clinical information and evaluate its impact on the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care.
• Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
• Assist with discharge planning and maintain knowledge of community resources.
• Participate in care conferences, meetings, and quality improvement initiatives.
• Maintain clinical competence through continuing education opportunities.
• Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.

• Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.

Ahava Therapy Services is hiring a Full-Time Occupational Therapist for Henderson Health & Rehab, a 137-bed skilled nursing facility that provides short- and long-term care and outpatient services. Therapists at Henderson work as part of an in-house collaborative rehab team, with strong leadership support, to help residents achieve independence and optimal function.
