Job summary

Ahava Therapy Services is seeking an Occupational Therapist for PRN coverage at Adamsville Healthcare & Rehab Center in Adamsville, TN. You will assess residents, develop individualized treatment plans, and collaborate with a multidisciplinary team to maximize function and quality of life.

You will supervise therapy assistants, document progress, and participate in care conferences while adhering to HIPAA and payer guidelines. A state OT license and BLS/CPR are required.

Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy
• Active OT license in the state of practice
• BLS/CPR certification (current or obtained during orientation)
• Strong clinical reasoning, communication, and documentation skills
• Ability to collaborate effectively within an interdisciplinary team

Responsibilities

• Review clinical information and evaluate its impact on the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care.
• Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
• Assist with discharge planning and maintain knowledge of community resources.
• Participate in care conferences, meetings, and quality improvement initiatives.
• Maintain clinical competence through continuing education opportunities.
• Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
• Support the company’s mission, vision, and professional expectations.
• Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.
