Job summary

Ahava Therapy Services is seeking an Occupational Therapist to join our team at Peachtree Centre in Gaffney, SC. You will assess residents, develop and implement individualized treatment plans, document progress, and collaborate with an interdisciplinary team to maximize functional outcomes.

We value compassionate care, ongoing education, and adherence to HIPAA and payer guidelines. A state license and BLS/CPR certification are required; bachelor's or master's in OT.

Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy
• Active OT license in the state of practice
• BLS/CPR certification (current or obtained during orientation)
• Strong clinical reasoning, communication, and documentation skills
• Ability to collaborate effectively within an interdisciplinary team

Responsibilities

• Review clinical information and evaluate its impact on the assessment and treatment process.
• Conduct comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Supervise and guide therapy assistants, ensuring proper treatment techniques and care delivery.
• Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
• Assist with discharge planning and maintain knowledge of community resources.
• Participate in care conferences, meetings, and quality improvement initiatives.
• Maintain clinical competence through continuing education opportunities.
• Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
• Support the company’s mission, vision, and professional expectations.
• Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.

Peachtree Centre is a 151-bed skilled nursing facility with 40 short-term rehab beds.
