Job summary

Best Home Health Providers, Inc. is seeking an Occupational Therapist to support patients recovering at home across the Bay Area and beyond.

You will evaluate functional abilities, develop treatment plans, and help patients regain independence in daily activities, aiming for safe home navigation. This per diem role offers flexible scheduling, mileage reimbursement, and opportunities to convert to full-time based on performance.

Qualifications

• Graduate of an occupational therapy curriculum accredited by ACOTE/CAAHEA.
• Registered with the NBCOT/National Registration Examination of the American Occupational Therapy Association.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license with reliable insured vehicle and ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in activities of daily living affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $110 - $140 per visit.

• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
