Job summary

Best Home Health Providers Inc. is seeking an Occupational Therapist to provide per diem home health services across the Bay Area. The role involves evaluating patients at home, developing individualized treatment plans, and guiding families and caregivers in ADLs.

You will travel to patient homes, coordinate with the care team, and help with discharge planning. The ideal candidate has CA OT licensure, 2+ years of OT experience, CPR certification, and a valid driver's license.

Qualifications

• Graduate of an occupational therapy curriculum accredited in the USA.
• Registered with the National Registry Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license and insured vehicle for home visits

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in activities of daily living
• Recommend adaptive equipment, splints, and orthotic devices
• Maintain accurate clinical records and report changes in patient condition
• Coordinate with the care team to ensure seamless patient-centered care
• Educate family members and caregivers on the treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition home

Please note that this role is per diem with a rate of $110 - $120 per visit.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns

• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
