Job summary

Best Home Health Providers, Inc. is hiring an Occupational Therapist to help patients regain independence in their homes across the Bay Area and beyond.

This per diem role focuses on evaluating patients, developing treatment plans, and educating families, with a competitive per-visit rate of $130. Candidates must be licensed in California, have 2+ years of clinical OT experience, and be willing to travel to patient homes with reliable transportation.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and AOTA.
• Registered with the National Registration Examination of the AOTA.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver's license with a reliable insured vehicle and ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient's treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
