Job summary

Best Home Health Providers, Inc. is seeking an Occupational Therapist for a per diem role providing in-home care across the Bay Area.

You will evaluate patients in their home, develop and communicate treatment plans, and retrain patients in activities of daily living to promote independence. Role involves travel to patient homes, supervision of OT Assistants when OT is the sole skilled service, and participation in discharge planning.

Qualifications

• California-licensed OT
• Registered with national OT board (AOTA)
• CPR (AHA) certified

Responsibilities

• Evaluate patients' functional abilities and home safety
• Develop personalized OT treatment plans
• Teach and retrain patients in daily living skills
• Instruct on adaptive equipment and splints
• Maintain accurate clinical records
• Coordinate with care team for seamless patient care
• Educate family members and caregivers on treatment approach
• Supervise OT Assistants and oversee home health aide care when OT is sole skilled service
• Support discharge planning during transition from home health care
• Perform other duties as required

Responsibilities

• Evaluate patients? functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient's treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

Requirements

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
