Job summary

Best Home Health Providers, Inc. is seeking a per diem Occupational Therapist to support patients returning home from medical facilities across the Bay Area.

You will evaluate home environments, develop treatment plans, and guide patients in daily living activities to restore independence and safety in the home. Responsibilities include coordinating with physicians and caregivers, supervising OTAs, and assisting with discharge planning.

Qualifications

• Graduate from ACOTE-accredited occupational therapy program.
• Registered or eligible for NBCOT; active California OT license.
• CPR certification (AHA) currently valid.
• Minimum 2 years of clinical OT experience.
• Valid driver's license and reliable insured vehicle; ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients' functional abilities and home environments for needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient's treatment approach.
• Supervise OTAs and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $130 per visit.

Requirements

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
