Job summary

LiveWell Homecare Agency, LLC is seeking an occupational therapist to provide skilled OT services to patients across home, school, and community settings in Vermont.

You will assess, plan, and implement interventions; collaborate with a multidisciplinary team; educate patients and families; and participate in quality improvement initiatives within a dynamic homecare environment.

Qualifications

• Bachelor’s degree or higher in an accredited OT program.
• Licensed or eligible for licensure in the relevant state.
• Strong understanding of physical, cognitive, psychosocial, and sensory factors impacting daily activities.
• Excellent communication and patient education skills.
• Ability to work effectively within a team and adapt to various clinical settings.
