Job summary

Live Well Homecare is seeking a licensed occupational therapist to assess, plan, and implement rehabilitative programs that improve patients’ daily activities, mobility, cognition, strength, coordination and range of motion in a home-based care setting. Your work supports quality outcomes, cost-effectiveness, and regulatory compliance.

You will coordinate with physicians to develop treatment goals, conduct patient assessments, educate patients and families, document evaluations and discharge

Qualifications

• Bachelor's or Master's degree in Occupational Therapy.
• Valid occupational therapist license.
• BLS certification.
• Effective communication, customer service orientation, teamwork, and basic computer skills are essential.

• Treatment planning and care: Implements and adjusts treatment plans, educates patients and families, provides discharge instructions, and refers to community resources.
• Patient assessment: Collaborates with physicians to develop and evaluate treatment programs, establish goals, and create realistic discharge plans.
• Documentation: Records evaluations, treatment sessions, and discharge information according to departmental guidelines.
• Professional and customer service: Upholds ethical standards, maintains confidentiality, demonstrates effective communication, fosters teamwork, and applies service excellence principles.
• Professional development: Participates in ongoing education, completes required competencies, and supports training of new staff.
