Evaluate residents and establish individualized occupational therapy plans of care.
Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
Assess and recommend adaptive equipment and environmental modifications.
Reassess patient progress and modify treatment plans as appropriate.
Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
Complete timely and accurate documentation using the electronic medical record (EMR).
Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
Current State Occupational Therapist (OT) License
Current CPR/BLS Certification
Valid driver's license, reliable transportation, and auto insurance
Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
