Job summary

Total Care Therapy is seeking a compassionate Occupational Therapist to join our therapist-owned team in Avon, IN. You will assess residents and craft individualized treatment plans to improve independence in ADLs, cognition, and safety.

Qualifications

• Current State OT License is required.
• CPR/BLS certification is required.
• Valid driver’s license with reliable transportation.
• Experience in geriatrics, home health, assisted living or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and establish individualized OT plans of care.
• Provide skilled OT interventions to improve ADLs, IADLs, upper extremity function, cognition, balance and safety.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess progress and modify treatment plans as needed.
• Supervise COTAs per state regulations and complete EMR documentation.
• Educate residents, caregivers, and staff on safety and energy conservation techniques.
• Collaborate with the interdisciplinary team to provide coordinated, patient-centered care.

About the Role

As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Qualifications

• Current State Occupational Therapist (OT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
