Job summary

Yale New Haven Health System seeks an Advanced Practice Provider (NP or PA) to join our interdisciplinary care team in Connecticut across inpatient, outpatient, and ambulatory settings. This role supports top-of-licensure practice with credentialing assistance and opportunities for growth within our integrated health system.

Responsibilities include comprehensive assessment, diagnosis, treatment, and ongoing management, collaborating with supervising physicians, and maintaining thorough EMR

Qualifications

• Graduate of an accredited NP or PA program.
• Master's degree or higher required, consistent with licensure.
• Experience as NP/PA preferred but not required.

Responsibilities

• Provide comprehensive assessment, triage, diagnosis, treatment, and ongoing management across settings.
• Prescribe medications within scope of practice and regulatory guidelines.
• Collaborate with physicians, nurses, and allied health professionals for patient care.
• Maintain accurate clinical documentation in the electronic medical record.
• Participate in quality improvement and credentialing processes.
