Job summary

Yale New Haven Health System is seeking an experienced Nurse Practitioner or Physician Assistant to support urogynecology care across outpatient clinics. You will evaluate, diagnose, and manage patients in collaboration with supervising physicians, maintain EMR documentation, and partake in care coordination across multiple locations in Connecticut.

The role emphasizes patient education, adherence to clinical guidelines, and ongoing professional development within a leading health system.

Qualifications

• Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
• APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS hospital and go through Medical Staff credentialing.
• Active PA Certification by NCCPA, CT license, ACLS/BLS, and DEA as applicable.

Responsibilities

• Provide outpatient urogynecology evaluation, assessment, diagnosis, and management.
• Obtain detailed histories, perform examinations, and develop treatment plans with supervising physicians.
• Order, interpret, and communicate diagnostic testing, labs, and imaging results.
• Prescribe medications and therapies per state regulations and guidelines.
• Perform office-based procedures within scope and competency.
• Coordinate care across multiple clinic locations in Lower Fairfield County as needed.
• Educate patients about diagnoses, treatments, medications, and postoperative care.
• Maintain accurate EMR documentation and ensure regulatory compliance.
• Participate in quality improvement and professional development initiatives.
