Sea Mar Community Health Centers is hiring an OB Coordinator for the Salmon Creek Medical Clinic. This full-time role runs Monday through Friday, 8:00 am to 5:00 pm, and focuses on OB tracking from pregnancy confirmation through six weeks postpartum, coordinating with the clinical team to address care needs.

The position emphasizes high-quality, patient-centered services. The OB Coordinator will participate in daily huddles, support patients, and connect them with Sea Mar and external resources,

Formación

• Must be able to work independently, prioritize workload, and meet deadlines.
• Knowledge of medical terminology and/or behavioral health topics.
• Excellent organizational skills and ability to handle a variety of tasks simultaneously.
• Strong decision making and prioritization skills.
• Ability to work respectfully and professionally with the community, patients, families and staff.

Responsabilidades

• Perform the prenatal intake and obtain past medical history.
• Document the information gathered during the intake process into the EPIC.
• Provide updates and information regarding shared OB and MSS clients when relevant.
• Assist patients with pregnancy management and link to social support services.
• Manage referrals, scheduling, and tracking to ensure patient attendance.
• Participate in weekly interdepartmental MSS and OB provider meetings.
• Complete prenatal contracts and intake orientation.
• Follow up with patients for planned visits and outcomes.
