Job summary

Community Health Center of Snohomish County is seeking an OB Coordinator to manage pregnancy care tracking from intake to six weeks postpartum. You will review daily reports, coordinate referrals, and support the clinical team to meet care measures in a patient-centered medical home framework.

Qualifications

• Reads, speaks, understands and writes proficient English.
• Ability to work independently and in a team.
• Proficient with Microsoft Office (Word, Excel, Outlook).

Responsibilities

• Screen patients for appropriateness of care with coordination of external referrals, as needed.
• Coordinate patient assignment to provider panels.
• Perform initial prenatal intake including past medical history.
• Assist patients with management of pregnancy care needs and social support services.
• Review educational handouts with patients and route questions to staff.
• Attend OB related team meetings.
• Register patients for hospital delivery and schedule C-section procedures.
• Maintain OB program statistics and quality measures.
• Monitor patient adherence to the plan of care and provide outreach.
• Prepare monthly reports for supervisors and providers.
• Data entry and patient chart review.
