Job summary

Sea Mar Community Health Centers is seeking a Care Coordinator to join our Clinical Care Team in Washington. This role focuses on coordinating care for patients with chronic conditions and behavioral health needs at the point of care.

You will participate in daily huddles, identify patient needs, provide short interventions, and link patients to behavioral health and other Sea Mar resources. The position emphasizes patient-centered, culturally aware services within a mission-driven organization.

Qualifications

• Medical Assistant training with 1+ year in community health.
• CPR certification within 90 days of hire.
• Bilingual Spanish preferred for the population served.
• Valid WA state driver’s license and proof of insurance.

Responsibilities

• Coordinate patient care and participate in team huddles.
• Identify gaps in care and support care transitions.
• Provide patient education and follow-up on care plans.
• Document and track patient information in EHR.
• Collaborate with clinic staff to ensure quality care.
