Qualifications

• Current nursing licensure in good standing.
• Experience in nursing leadership or wellness coordination.
• Strong clinical judgment and communication skills.
• Ability to build relationships with residents, families, and colleagues.
• Familiarity with care planning, documentation standards, and regulatory expectations.
• Demonstrated professionalism, empathy, and accountability.
• Experience with electronic documentation platforms is preferred.
• Ability to manage multiple priorities in a fast-paced setting.

Responsibilities

• Lead wellness and nursing-related programs in alignment with organizational policies and care standards.
• Assess resident health needs and coordinate appropriate care plans and follow-up activities.
• Partner with physicians, families, and care teams to support continuity of care.
• Monitor wellness indicators, identify concerns, and escalate issues when necessary.
• Support medication management processes and ensure compliance with applicable regulations and protocols.
• Supervise, coach, and support nursing or wellness team members as needed.
• Maintain accurate documentation and contribute to quality improvement efforts.
• Promote a respectful, engaging, and supportive environment for residents and team members.
• Assist with staff education, training, and wellness initiatives.
• Respond appropriately to urgent clinical situations and coordinate next steps.
