Job summary

The Shores of Lake Phalen, a Sonida Senior Living community, seeks a skilled RN or LPN to join our assisted living team in Maplewood, MN. You will perform resident assessments, support medication management, and ensure accurate charting while maintaining compliance with state regulations.

Qualifications

• Must be licensed as RN or LPN in MN.
• CPR and First Aid certification required or able to obtain within 90 days.

Responsibilities

• Conduct timely resident assessments and report changes.
• Oversee medication management and MAR accuracy with pharmacy support.
• Complete charting promptly per state rules.
• Communicate with families about resident status and events.
• Assess potential residents for eligibility of Assisted Living.
• Provide on-call backup and ensure confidentiality.
• Document incident reports accurately.

Registered Nurse (RN) or Licensed Practical Nurse (LPN) Responsibilities include:

• Involves being completely familiar with the state regulations governing Assisted Living and ensuring compliance
• Performs timely and accurate resident assessments utilizing assessment tool, and communicating changes to the Executive Director
• Provides oversight with medication management to assure all MAR's are correct and that the residents receive medications timely and accurately. Working with consultant pharmacist as needed to assure compliance
• Ensures all charting is performed in a timely manner and in accordance with state regulations.
• Provides timely and honest communication with families about the resident's condition and community events
• Provides assessments for potential residents to determine their eligibility for Assisted Living.
• Provides on-call backup to community when not on duty. Maintains confidentiality of all pertinent personal or health information concerning residents and staff
• Ensures that any incident reports are completed properly and promptly

Qualifications

• Qualified candidates must be currently licensed as a RN or LPN in MN with a minimum of 1-year experience in senior housing and/or geriatric nursing
• Candidate must either be certified or be able to become CPR and First Aid Certified within the first 90 days of employment

Wage:

$30 - $40 depending on experience
