Job summary

The Shores of Lake Phalen, a Sonida Senior Living community in Maplewood, MN, is hiring for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide high-quality care in Assisted Living and Memory Care settings. You will conduct assessments, oversee medication management, and work with families and the care team to support resident safety.

Qualified MN-licensed RN or LPN with 1+ year experience in senior housing/geriatric nursing; CPR and First Aid certification within 90 days

Qualifications

• Licensed RN or LPN in MN with at least 1 year experience in senior housing/geriatric nursing
• Must obtain CPR and First Aid certification within first 90 days of employment

Responsibilities

• Familiar with state regulations governing Assisted Living and ensure compliance
• Perform timely resident assessments using designated tools and communicate changes to the Executive Director
• Oversee medication management to ensure timely and accurate MARs in coordination with a consultant pharmacist
• Ensure timely charting in compliance with state regulations
• Communicate honestly with families about resident condition and events
• Assess potential residents for eligibility for Assisted Living
• Provide on-call backup as needed and maintain confidentiality of health information
• Complete incident reports properly and promptly

**Eligibility based on location

Registered Nurse (RN) or Licensed Practical Nurse (LPN) Responsibilities include:

• Involves being completely familiar with the state regulations governing Assisted Living and ensuring compliance
• Performs timely and accurate resident assessments utilizing assessment tool, and communicating changes to the Executive Director
• Provides oversight with medication management to assure all MAR's are correct and that the residents receive medications timely and accurately. Working with consultant pharmacist as needed to assure compliance
• Ensures all charting is performed in a timely manner and in accordance with state regulations.
• Provides timely and honest communication with families about the resident's condition and community events
• Provides assessments for potential residents to determine their eligibility for Assisted Living.
• Provides on-call backup to community when not on duty. Maintains confidentiality of all pertinent personal or health information concerning residents and staff
• Ensures that any incident reports are completed properly and promptly

Qualifications:

• Qualified candidates must be currently licensed as a RN or LPN in MN with a minimum of 1-year experience in senior housing and/or geriatric nursing
• Candidate must either be certified or be able to become CPR and First Aid Certified within the first 90 days of employment

Wage: $30 - $40 depending on experience
