Regal Heights Nursing Center is seeking an experienced Social Worker to join our caring team. We value a compassionate professional who can support residents' social and emotional needs and coordinate with families and clinical staff.

The ideal candidate has nursing home or hospital experience and a BSW or MSW. English/Spanish bilingual abilities are a plus. We offer a supportive work environment and meaningful opportunities to improve residents' quality of life.

Qualificações

• Nursing Home or Hospital experience. Internship is sufficient. (Required)
• BSW or MSW in related field. (Required)
• Bi-lingual English/Spanish (preferred but not required)

Responsabilidades

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Initiating appropriate interventions for residents experiencing mental or behavioral health issues
• Identifying individual social and emotional needs
• Participate in the planning of the resident’s admission, community re-entry, or transfer to another facility
• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay
• Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community

Conhecimentos

Nursing Home experience

MSW/BSW degree

English/Spanish (bilingual)
