Job summary

Regal Heights Nursing Center is seeking an experienced Social Worker to join our caring team at a nursing home setting in New York. The role requires a BSW or MSW and experience in a nursing home or hospital environment. Bilingual English/Spanish is a plus.

The position emphasizes supporting residents' psychosocial needs, coordinating admissions and discharges, and collaborating with families and the care team to ensure smooth transitions.

Qualifications

• Nursing Home or Hospital experience is required.
• BSW or MSW in related field is required.
• Bi-lingual English/Spanish is preferred but not required.

Responsibilities

• Maintaining and improving residents' ability to manage daily physical needs and psychosocial well-being.
• Initiating appropriate interventions for residents with mental or behavioral health concerns.
• Identifying individual social and emotional needs of residents.
• Participating in planning for admission, community re-entry, or transfer to another facility.
• Collaborating with the finance team to educate residents and ensure proper documents are in place.
• Coordinating discharge planning and arranging home care or DME as needed.

Qualifications:

• Nursing Home or Hospital experience. Intership is sufficient. (Required)

Social Worker Duties and Responsibilities:

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs

• Initiating appropriate interventions for residents experiencing mental or behavioral health issues

• Identifying individual social and emotional needs

• Participate in the planning of the resident’s admission, community re-entry, or transfer to another facility

• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay

• Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community
