Job summary

Regal Heights Nursing Center is seeking an experienced Social Worker to join our caring team. The role involves supporting residents’ physical and psychosocial needs, coordinating admissions and transfers, and ensuring proper documentation and discharge planning.

Internship is acceptable for experience requirements, and bilingual English/Spanish is a plus. The position emphasizes resident advocacy, collaboration with the finance team, and creating a supportive, patient-centered environment

Qualifications

• Nursing Home or Hospital experience is required; Internship is sufficient.
• BSW or MSW in related field is required.
• Bi-lingual English/Spanish is preferred but not required.

Responsibilities

• Maintaining and improving residents' physical and psychosocial needs.
• Initiating interventions for residents with mental or behavioral health issues.
• Identifying individual social and emotional needs of residents.
• Participating in admission planning, community re-entry, or transfers to other facilities.
• Collaborating with the finance team to educate residents and ensure proper documentation.
• Coordinating discharge planning and arranging DME or home care for safe community return.

Qualifications:

• Nursing Home or Hospital experience. Intership is sufficient. (Required)

Social Worker Duties and Responsibilities:

• Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needs

• Initiating appropriate interventions for residents experiencing mental or behavioral health issues

• Identifying individual social and emotional needs

• Participate in the planning of the resident's admission, community re-entry, or transfer to another facility

• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay

• Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community
