Legacy Healthcare is seeking a Licensed Nursing Home Administrator (LNHA) to lead a skilled nursing facility in Peoria, IL. The LNHA will drive overall operations, regulatory compliance, and financial performance while ensuring high-quality, resident-centered care in a safe and compliant environment.

The role requires demonstrated leadership in long-term care, strong knowledge of CMS/state surveys, and effective management of staff, budgets, and quality initiatives.

Qualifications

• Current and unrestricted LNHA license in the applicable state.
• Bachelor’s degree preferred in healthcare administration, business, nursing, or related field.
• Previous leadership experience in skilled nursing or long-term care.
• Strong knowledge of federal/state nursing facility regulations and survey processes.
• Experience managing staff, budgets, quality initiatives, and facility operations.
• Excellent communication, leadership, organizational, and problem-solving skills.
• Ability to work with residents, families, employees, physicians, and regulatory agencies.
• Proficiency with electronic medical records and standard software.

Responsibilities

• Provide leadership and day-to-day management of the nursing facility.
• Ensure compliance with CMS, state regulations, and licensing requirements.
• Maintain resident-centered care, safety, dignity, and quality.
• Lead department directors and interdisciplinary teams.
• Monitor metrics and implement corrective actions.
• Develop and evaluate facility policies and procedures.
• Prepare for regulatory surveys, audits, and investigations.
• Manage annual operating budget, census goals, labor costs, and financial performance.
• Collaborate with nursing leadership to promote quality outcomes and compliance.
• Oversee staffing, recruitment, retention, and employee engagement.
• Address resident and family concerns promptly.
• Build relationships with residents, families, physicians, hospitals, vendors, and community partners.
• Oversee QAPI initiatives and performance improvement activities.
• Ensure accurate documentation and maintenance of records.
