Qualifications

Responsibilities

• Utilizes the nursing process to develop, implement, and evaluate case management outcomes for defined patient populations
• Educates, consults, and assists in the management of the identified patient population
• Collaborates with the patient care team to achieve optimal patient outcomes and cost-effective care
• Available by pager, phone, and e-mail when off-campus
